2-digit dependent code: 00 for the member (i.e. the student), 01 for the member’s spouse (21, 31, etc. for subsequent spouses), and 02-20 for dependent children (oldest is 02, second oldest is 03, and so on)

Health and dental claims

4 simple claiming options

Option 1: Pay-Direct Card

Present your Pay-Direct Card to pharmacists, dentists, and a number of other health professionals for immediate reimbursement (you won’t have to pay the full amount of your bill up front).

Option 2: Mobile app

Submit your claims through the GreenShield+ mobile app.

Option 3: Online portal

Option 4: Claim forms

Mail a claim form to GreenShield at the address indicated on the form.

Claims deadline

All health and dental claims must be received by GreenShield no later than 12 months after the date the eligible benefit was incurred. Be sure to leave adequate time for delivery.
