3 simple claiming options

Mobile app (health claims only). Download the Pacific Blue Cross mobile app on the App Store or Google Play to submit claims through your Member Profile.

Online (health claims only). Register for your Member Profile online and claim through the Pacific Blue Cross website.

Mail. Download and complete a Claim Form, and submit it to the address on the form. Be sure to leave enough time for delivery!

*Note that dental claims can only be submitted by mail.

Claims deadline

The insurer must receive your claims no later than:

• 90 days after the end of the policy year in which the claims were
incurred.

• 90 days after the end of your coverage

Whichever date is sooner will apply. Full-year coverage ends Aug. 31, 2023.

Note: Reimbursement is based on reasonable and customary fees. The benefit you're claiming must be paid in full and be in your possession before the end of the policy year.
