HEALTH, VISION & DENTAL CLAIMS
If you're submitting claims for services incurred before Sept. 1, 2024, your claims must be received by Desjardins Insurance by Nov. 29, 2024.
Submit your claims via the secure online submission tool to avoid mail delivery delays.
Use the claim forms found below (please ensure you fill out the form completely):
You’ll need your Group Number and your Certificate Number to submit claims:
• Group Number (health, vision, and dental): Q1109
• Certificate Number: Your 9-Digit Student ID
TRAVEL CLAIMS

GENDER AFFIRMATION CARE CLAIMS
If you’re submitting claims for gender-affirming care procedures incurred before Sept. 1, 2024, your claims must be received by GreenShield by no later than 12 months from the date the eligible benefit was incurred.
Complete the GreenShield Health Claim Form and mail it to GreenShield, along with receipts for your procedures and diagnosis of gender dysphoria, at this address:

You’ll need your Group Number/Prefix and your Member ID Number to submit claims:
• Group Number/Prefix: GAP • Member ID Number: GAP + your student ID number + your 2-digit dependent code
The dependent code is 00 for the member (i.e. the student), 01 for the member’s spouse (21, 31, etc. for subsequent spouses), and 02-20 for dependent children (oldest is 02, second oldest is 03, and so on).
